China suggests Jack Daniels as a cure for corona virus

How to handle sputum and mucus in small airways of patients with novel coronavirus pneumonia?

This study provide us a unique opportunity to understand the clinical characteristics of patients infected SARS-CoV-2, these results will further provide physicians with critical references for the follow-up choice of treatment schedule.1 Bypass these useful clinical evidences, several recent reported pathological findings indicated that NCP patients is associated with fiber-mucus like exudation and pulmonary edema in lung tissue. 2-4Therefore, I considered airway management and oxygen supply as the life-sustaining treatments are particularly important.
To our knowledge, oxygen supply as adjuvant therapy is widely used in patients with respiratory and cardiovascular diseases. For patients with novel coronavirus pneumonia (NCP), oxygen supply is considered an important part of comprehensive treatment, and improved oxygenation index commonly reflects the recovery of respiratory function and remission of the disease. Nasal catheter/mask, non-invasive and invasive ventilation are general approaches for oxygen supply. However, some potential factors may affect the efficacy of oxygen supply. Firstly, anxiety status in the course of ventilation may trigger the nervous reflex so as to aggravate the spasm of microvasculars and small airway, which eventually lead to an increase in respiratory and circulatory resistance. Secondly, the sputum with mucus existing in the deep end of the small airway is not easy to be sucked out by conventional methods, which might directly affect the efficacy of mechanical ventilation. Meanwhile, the sputum will further promote imbalance of ventilation/ perfusion ratio in local tissue of lung. Thirdly, sputum deposition in bronchi and bronchioles will increase the risk of double infection or virus reproduction. Lastly, novel coronavirus pneumonia can cause local pulmonary edema, which can rise surface tension of alveoli and hinder the gas exchange, and this condition may be fatal in patients with cardiac dysfunction. Although, hormone therapy can reduce inflammation and exudation to some extent, its applicability remains to be defined due to data shortage, glucocorticoid induced inhibition of immune capacity and the following upgraded risk of virus proliferation should be considered. All these issues mentioned above are urgent to be solved.
To make it easier to conduct suctioning and care by doctors and nurses, experiences in the treatment of acute pulmonary edema provides us clues that alcohol humidified-oxygen supply may be favorable for gas exchange. As we know, method of alcohol humidified-oxygen supply is commonly used for patients with acute pulmonary edema due to the antifoaming action.5-7 Pulmonary edema patients with excessive exudation in the alveoli will produce a large amount of foam sputum. These foam sputum accumulate in the alveoli, which mixed with mucus will lead to dyspnea and suffocation. Alcohol can effectively reduce the surface tension of alveolus and eliminate the foam, thereby improving the patient's dyspnea, relieving anxiety and respiratory muscles fatigue.
Clinical and experimental evidences suggested that the concentration of alcohol used for humidifying oxygen ranged from 20 to 95 percent, the general concentration of alcohol is from 20 to 30 percent.5In some reported cases, a nasal catheter and 95 percent alcohol is the most suitable for conscious patients, a mask and 30 to 40 percent alcohol is more convenient in unconscious patients6, and adverse reactions had rarely been reported excepting few allergic cases. Based on these experiences mentioned above, humidified-oxygen supply seems a useful method for diluting sputum, it may be beneficial for sputum excretion and alleviation of airway obstruction, and this method is also suitable for patients with mild or moderate NCP.
Novel coronavirus pneumonia outbreak in Wuhan constitutes considerable challenges for Chinese doctors and nurses. The therapeutic efficacy not only depends on optimal medical resources and specific drugs, but also scientific nursing. For treating NCP patients, sputum suction and oxygen supply form one therapeutic unit, although the sputum suction is a content of nursing. Effective sputum suction provides an airway support for the follow-up oxygen supply. The smooth process of sputum suction will not only benefit NCP patients, otherwise the smooth duration of suctioning will be helpful for degrading the risk of the infection among health care workers.
